Overview

The Population Health Specialist (PHS is an essential administrative member of our dynamichealthcare team, focused on supporting the activities of the Care Coordination Department through patient outreach and various clerical duties.  Excellent time management and an ability to work as part of a team are essential skills for this role.

 

Location: On-site Verona, NJ office, 7am – 3pm, with occasional travel to other VMG sites or for professional development training

Department: Care Coordination

Reports to: Director of Care Coordination

Responsibilities

  • Accurate data management including the retrieval of hospital records and obtaining reports from specialists and health care providers
  • Manage data entry on multiple spreadsheets to track workflows and outcomes
  • Manage Emergency Room spreadsheet and workflow
  • Participate in Population Health initiatives including managing outreach programs, mailings, and patient education programs
  • Attend trainings, meetings and presentations associated with the Care Coordination Department and the VMG organization as required
  • Perform administrative duties including copying, report collation, organizational tasks, and other duties as assigned by Director

Qualifications

  • Work experience in medical health care setting using electronic health care record (EHR) required; college degree preferred
  • Excellent communication skills (written and verbal) and the ability to build trusting relationships with colleagues and patients
  • Empathy, compassion, and an understanding of various cultural and socioeconomic backgrounds in the community;  a creative and resourceful problem-solver
  • Effective time-management skills required; ability to independently complete tasks in a busy medical office environment efficiently and accurately
  • Strong skills in Word, Excel, and ability to work with electronic health records

 

 

Compensation and Benefits:

 

The annualized anticipated base annual salary for this Population Health Specialist ranges from $47,500 to $49,900. Additionally, this position offers an opportunity for annual bonus, and other benefits.

 

Other benefits offered include a 401K retirement savings plan, paid time off (vacation/personal days, holidays, and sick time), and health benefits (medical, prescription drug, dental and vision insurance). 

 

Please note that individual total compensation for this position will be determined at the Company’s sole discretion and may vary based on several factors, including but not limited to location, skill level, years and depth of relevant experience, and education.
